Policies:
AT EVERY VISIT:
Please bring the patient’s insurance card, your photo-ID and any payment/co-pay applicable at each visit.
Federal trade commission identity theft: ‘RED FLAGS RULE’: We have to check photo identification at every office visit to comply with this rule.
APPOINTMENTS:
When making your appointment over the phone, we ask that you please have your current medical insurance information available for our staff. After collecting some brief information, the staff will assist you in making an appointment.
Please schedule the next physical exam or follow up appointment before you leave.
Walk-in appointments:
We prefer that you call us so we can schedule your child at a convenient time so you do not have to wait. However, a schedule appointment will take priority over walk-ins,
We realize that your time is valuable and we will make every effort to honor time commitments. When our providers encounter problems or emergencies, we ask for your patience and understanding.
To make every appointment worthwhile, please bring a list of all currently prescribed medications, with their strength and dosage. Before each appointment, write down any questions or concerns that you would like to discuss. Always ask us to explain anything you don’t understand. Take notes. Writing down instructions will help you remember them after you leave.
IMMUNIZATIONS:
We follow the guidelines set by the American Academy of pediatrics. We believe that immunizations are essential and necessary. We no longer accept patients who do not immunize.
FLU VACCINE:
Please call us in September to schedule your flu vaccine.
CALL REMINDERS:
A courtesy call is given prior to the appointment but we expect the caregivers to make note of their appointments.
FAILURE TO KEEP APPOINTMENTS:
A fee will be charged for any missed appointments or for failure to cancel the appointment 24 hours prior to the appointment.
We shall be unable to schedule any more appointments if either the initial appointment or more than 3 appointments are missed.
INSURANCE COVERAGE:
We accept most insurance carriers. Please have your card with you at the time of your visit. If we file your claim, you are responsible for the co-insurance or the amount set by your insurance if you have a deductible. All CO-PAYS are expected at the time of the visit. We will be charging an administrative fee if asked to the bill. If we are not a provider for your insurance, if you are not insured or we do not file with your insurance company, you may be responsible for the entire charge at the time of service.
RETURNED CHECKS:
An administrative fee of $25.00 or more will be charged for every returned check.
PRESCRIPTION REFILLS AND FORMS:
Please give us 24-48 hours. Forms not requested at the annual physical exam will be charged an administrative fee of $5.00 each.
QUESTIONS AT THE VISIT:
If you are here for a specific visit such as for a vaccine, please understand that any concern expressed and addressed has to be documented as an office visit. Please let us know earlier if you have any other concerns besides the vaccine administration.
ACCOMPANYING CHILDREN:
Please let us know when you come, if the accompanying child(ren) also need to be seen so we can inform you if it is possible, without inconveniencing the next patient.
ELECTRONIC DEVICES:
Please do not use any electronic devices while in the office. Our patients and providers need your full attention and we request all to respect that. Thank you for understanding and please let us know if you have any questions.

Reach out and Read
- Reach Out and Read is a non-profit organization that gives young children a foundation for success by incorporating books into pediatric care and encouraging families to read aloud together.
- At certain well child visits, our providers will discuss the importance of making reading aloud with your child a daily habit. We will also provide your child with age-appropriate books and share advice on how to effectively engage with your child as you read aloud together. Learn more about Reach out and Read.
Childhood Literacy and Development
As part of our commitment to Early Childhood Development, we are partnered with the Reach Out and Read (ROR) program. This emphasis on literacy encourages and helps children develop critical, early reading skills. Our practice follows the American Academy of Pediatrics guidelines, and incorporates literacy checks, right into the regular office visit. We promote the importance of reading to parents and children, by providing books to patients. We believe this approach ensures that each child gets an early start to reading and also creates an opportunity for families to bond over a shared activity.

NCQA’s Patient-Centered Medical Home (PCMH) Recognition program is the most widely adopted PCMH evaluation program in the country.The patient-centered medical home is a model of care that puts patients at the forefront of care. PCMHs build better relationships between patients and their clinical care teams. The Patient-Centered Medical Home (PCMH) is a care delivery model whereby patient treatment is coordinated through their primary care physician to ensure they receive the necessary care when and where they need it, in a manner they can understand.
A PCMH places the patient in the spotlight and encourages the development of a true medical home. It requires providers to strengthen relationships with their patients, families, local hospitals, specialty providers and with their own staff members. Every team member is critical to the success of a medical home.
The PCMH model provides higher quality coordinated care and communication for the patient. The structure enables the primary care physician to develop a long-term relationship with the patient as well, which has been proven to improve the quality of care provided to that patient.
